2016 Application

The Diabetes Scholars Foundation’s college scholarship program is available to incoming freshmen with type 1 diabetes seeking a higher education at an accredited four year university, college, technical or trade school. This scholarship recognizes students who have high academic performance, are actively involved in the diabetes community, participate in community and/or extra-curricular activities and who have demonstrated that they are successfully managing the challenges of living with diabetes. Diabetes Scholars Foundation scholarships are not based on financial need.

Eligibility:

  • High School Seniors with Type 1 diabetes seeking a higher education at an accredited four year university, college, technical or trade school.
  • U.S. citizen or permanent resident.

Scholarship Awards:  

The following scholarships will be awarded to students who are high school seniors and will be incoming freshmen in the fall and who have Type 1 diabetes:

      • The Lilly Diabetes Tomorrow’s Leaders Scholarship – eleven scholarships at $5,000 each, five scholarships at $4,000 each and five scholarships at $3,000 each will be awarded to students that will be incoming freshmen with type 1 diabetes
      • Novo Nordisk Scholarship – four $5,000 scholarships will be awarded to students that will be incoming freshmen with type 1 diabetes
      • Jay Cutler Athletic Scholarship – five $5,000 scholarships will be awarded to a student that will be an incoming freshmen with type 1 diabetes and will be playing a competitive sport in college.
      • Type 1 for 67 years so far – three $5,000 scholarships will be awarded randomly to an incoming freshmen type 1 diabetes
      • The Jay Franke Scholarship – two $5,000 scholarships will be awarded to an incoming freshmen with type 1 diabetes and  pursuing an Arts Degree (music, theatre, dance)
      • The Bradley D. Gendron Memorial Scholarship – one $1,000 scholarship will be awarded to an incoming freshmen with type 1 diabetes residing in Colorado
      • The Josh Smith Memorial Scholarship – one $1,000 scholarship will be awarded to an incoming freshmen with type 1 diabetes and residing in Ohio
      • Gaspari Science Scholarship – one $1,000 scholarship will be awarded to an incoming freshmen with type 1 diabetes and pursuing a Science degree
      • The Joseph Mahoney Memorial Scholarship – one $2,500 scholarship will be awarded to a high school junior or senior with type 1 diabetes who is attending West Chicago High School in West Chicago, Illinois
      • Mayor Ruben Pineta Community Scholarship – one $2,500 scholarship will be awarded to a high school junior or senior with type 1 diabetes who is attending West Chicago High School in West Chicago, Illinois
      • Chef Alain Roby Culinary Scholarship – one $5,000 scholarship will be awarded to an incoming freshmen from Illinois with type 1 diabetes pursuing a bachelor or associates degree in culinary arts from an accredited college or university
Award Announcements

      • The recipients of the 2016 scholarships will be telephoned no later than June 1, 2016. Those applicants not selected will not receive any other notification
      • Checks will be distributed directly to the school in July once it’s been verified that the recipients have been accepted and registered to an accredited four year university, college, technical or trade school
